Register online (Preferred)

Once you have your student address (you must register with your new local address), there are two steps to register online. We do require you to do both.

PreRegistration1. Click here to complete the online registration form. Please ensure you complete ALL sections including information about your medical history.

2. Click here and complete the form to provide some essential further information required to complete your registration. We cannot approve your registration until this form has been received. Once completed, email the form to docman.woodbridgehill1@nhs.net

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternative registration (On paper)

Alternatively you may print off a registration form and new patient questionnaire, fill them out and send them/post to the practice/drop into the letter box. If you register on paper, please can you also copy a form of identification such as passport, driving licence or copy of other typescript document that gives your name and date of birth. This is to avoid spelling errors that can occur with handwritten documents.
